Milwaukee County Department of Health and Human Services (DHHS) Disabilities Services
Division (DSD), in an initial partnership with iCare and Community Care, Inc., received a grant
in 2006 from the Wisconsin Department of Health and Family Services
(DHFS) to plan a reformed system of long-term care for adults with disabilities under the age of 60 in Milwaukee County. This is part of a statewide effort to plan a more integrated and better managed system of long-term care. One of the main goals is to reduce and eventually eliminate waiting lists for long-term care services. Milwaukee County
DHHS DSD, iCare and Community Care, came together as “Partners” in this long-term care planning process.
However, when the State grant money was expended and the model
for long-term care in Milwaukee County envisioned that the
existing Milwaukee County Family Care
Care Management Organization (CMO) would be expanded and the
Partners would offer other independent CMO choices, Milwaukee
County DHHS and DSD funded continued planning efforts and the
Partners became less involved.
The Planning Council for Health and Human Services, Inc. is providing project management services.
A plan for Milwaukee County's long-term care system is
expected to be ready in 2008. Actual implementation is not
expected until 2009.
